Shift Type/Shift Times/Hours/Day: Days 8:00 AM 5:00 PM 8

Shift Notes: 8:00 AM 5:00 PM/40 hours scheduled/32 hours guaranteed/Client is allowed to cancel up to 1 shift per week

Weekend Requirements: Weekend schedule to be determined by facility

OnCall Requirements: Oncall schedule to be determined by facility

Traveler Policy: n/a

• Learning modules must be completed prior to start of the assignment.
• Nonbillable hours have been adjusted to reflect 16 hours for each assignment.
• Client or Vendor may cancel an Assignment in progress with 7 days written notice without penalty.

This position is responsible for the evaluation planning administration and supervision of Physical Therapy treatment following physician referral state and national organizational standards regulatory agencies and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.

• Minimum 24 months of Inpatient/Outpatient PT experience REQUIRED
• Certifications must be American Heart Association OR American Red Cross and active upon and INCLUDED IN SUBMISSION REQUIRED
• No License Hits REQUIRED
• Active CO PT license required at time of submission REQUIRED
• will not approve RTO within the first two weeks or holidays. Can not exceed 7 days.
