We are seeking a Registered Nurse for our House Calls program. This is an excellent opportunity for an experienced and passionate medical professional to make a meaningful difference in the lives of our patients by providing them with highquality compassionate and comprehensive health care in the comfort of their own homes. The ideal candidate for this position should be a highly motivated and organized individual with an advanced degree in nursing and experience in a clinical setting as an RN.
The Nurse will be part of our growing medical professionals. Your primary responsibility will be to provide advanced primary and/or specialty care to our patients.
Responsibilities include:
- Conduct inhome assessments including an evaluation of past medical history review of systems medication reconciliation vital signs and comprehensive physical exam
- Collects records and maintains accurate patient information
- Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
- Perform evidencebased practice screenings including pointofcare testing (as appropriate)
- Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
- Communicate findings in your assessment that will be used to inform member s PCP of potential gaps in care
- Identify urgent and emergent situations and intervene appropriately
- Educate members on topics such as disease process medication and compliance
- Documents items such as past medical family and social history review of systems examinations medications allergies assessment and plan
- Orders or executes various tests analyses and diagnostic images to provide information on a patients condition
- Ensures fulfillment of our commitment for ensuring quality care
- Patientcentered care in collaboration with other health care providers and clinical staff to identify and address gaps in care.
Requirements
- Current New York State License as an RN
- National Board Certification required
- American Association of Nurse Practitioners (AANP)
- American Nurses Credentialing Center (ANCC)
- Bilingual (Spanish Speaking) is a plus
- EMR/EHR experience a plus
