RN / HOSPICE CASE MANAGER (hiring 3 people)
SPECIAL INSTRUCTIONS:Do not apply for this position unless you have hospice and / or home health experience. No exceptions. Thank you.
TERRITORY: Dallas/Mesquite/Garland/Plano/Lancaster/Desoto/McKinney/Plano areas.
POSITION SUMMARY:
Meet with patients and families to explain the scope of hospice services, coordinate admission of patient and carrying our ongoing plan of care.
ESSENTIAL FUNCTIONS
- Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME
- Case manage total care of patient by coordinating with team members/physicians, supervising C N A activities and participating in staff and Interdisciplinary Team meetings
- Complete daily paperwork and charting, thoroughly and accurately according to Policy
- Participate in on-call rotation
- May assist with admissions
POSITION QUALIFICATIONS:
Registered Nurse in the state of Texas
Hold a valid driver’s license and have a dependable means of transportation
KNOWLEDGE/SKILLS & ABILITIES
Strongly prefer:1+ years minimum experience in hospice case management
Other Requirements:Ability to think autonomously and make good decisions; have good communication skills, exhibit a confident demeanor that can remain calm in difficult situations.
