Our client is seeking an experienced NDIS Support Coordinator or Plan Manager looking to move into a Support Coordination role within an established and experience Support Coordination team. The successful applicant will:
- Manage a portfolio of approximately 30 clients / billing average over the year 30 to 34 hours a week.
- Support clients to understand their NDIS plan and the support options for achieving their goals.
- Ensure all reporting requirements are completed as per NDIS guidelines.
- Answer all enquiries from clients and stakeholders in a timely manner.
- Manage and maintain client records and information proficiently via online software.
- Triage new client referrals, when required, and seek alternative service providers if the company is not the best fit for the client.
- Share and contribute own unique skills and experience to continually grow the depth and breadth of the teams knowledge.
Requirements

- Minimum of 2 to 3 years experience in a NDIS Support Coordination or Plan Manager role.
- Tertiary qualifications in a relevant field social worker, psychology, allied health, mental health, community services, nurse, social or health science professional will be highly regarded.
- Experience coordinating support for people with complex disabilities,high intensity care including mental health and psychosocial disabilities, housing, leaving school is also highly regarded.
- Ability to undertake research, deliver information in an easy-to-understand format, draft clear and concise reports / written information and build key relationships with a range of diverse stakeholders.
- Must have an advanced level of written and verbal communication skills, be highly organised and be able to manage competing priorities whilst delivering consistently high-quality outcomes.
- Full competence with computer systems, apps, NDIS Portal and other technology relevant to the role.
