The Infection Prevention Re-engagement Specialist will be responsible for performing linkage to care services for people living with HIV (PLWH) who have fallen out of medical care greater than 12 months in the Eastern Region of Tennessee ( East, and Northeast Regions, including metros such as Knox County and Sullivan ).
Job will be base out of Knoxville, TN
Job responsibilities include:
Conduct re-engagement services for people living with HIV (PLWH) residing in the Eastern region, reconnecting those that have fallen out of care back into HIV medical care services
o Obtain a monthly list of persons with HIV identified as being out-of-care
o For each quarter, investigate PLWH identified as being out-of-care
Conduct thorough record searches
Conduct field visits
Make phone calls to providers and clients
Send letters to clients
o For each quarter, re-engage in or verify medical care services for PLWH who were identified as being out-of-care
Re-orient clients to the Ryan White insurance process
Schedule and attend provider visits
o For each quarter, track the number of PLWH who were re-engaged into medical care within 30 days and within 90 days
o Verify the number of PLWH who are re-engaged into medical care and retained in care three months or greater following linkage activities
o Verify the number of PLWH who are re-engaged into medical care and are virally suppressed within 12 months following linkage services
o Each day, track efforts made to re-engage PLWH using REDCap, CAREWare and other applicable databases
Successfully sustain partnerships with the local HIV medical care community to enhance HIV care management for persons living with HIV in the East Region
o Provide HIV Re-engagement Program information and guidance to HIV medical care providers, medical case managers, local health department staff, and community-based organizations at least once per year
o Continuously identify at least one staff person at each HIV medical care facility and community-based organization to serve as the main contact person for re-engagement related inquiries
Conduct additional case investigations such as provider referrals and cluster investigations outside of the assigned NIC list as needed and as appropriately directed
o Investigate persons identified as provider referrals and/or in a HIV molecular cluster
Conduct thorough record searches
Conduct field visits

Send letters to clients
o Verify the number of PLWH who are re-engaged into medical care and retained in care three months or greater following linkage activities
o Verify the number of PLWH who are re-engaged into medical care and are virally suppressed within 12 months following linkage services
o Track effoorts made to re-engaged PLWH using REDCap, CAREWare and other applicable databases
Attend re-engagement related meetings and events including the annual HIV Statewide meeting, weekly HIV Prevention team meetings, quarterly Re-engagement DIS meeting, and annual program audit
Complete re-engagement related trainings to enhance job performance and skill sets throughout the Re-engagement Program calendar year
Provide public speaking as appropriately directed
Required Education and Experience: Graduation from an accredited college or university with a bachelor's degree and experience equivalent to one year of full-time professional health program experience that involves the investigation, prevention, or treatment of individuals with communicable diseases.
Substitution of Experience for Education: Qualifying full-time professional health program experience that involves the investigation, prevention, or treatment of individuals with communicable diseases may be substituted for the required education, on a year-for-year basis, to a maximum of four years.
Substitution of Education for Experience: Additional qualifying graduate coursework in public health, public health education, epidemiology, nursing, or social work may be substituted for the required experience, on a year-for-year basis, to a maximum of one year (e.g., 36 graduate quarter hours in one or a combination of the above listed fields may substitute for one year of the required experience).

Experience collecting and managing data
Strong oral and written communication skills
Familiarity with HIV Prevention Services, disease intervention and disease investigation
Willingness to travel extensively throughout Tennessee
