Physical Therapists provide services to patients/clients who have impairments functional limitations disabilities or changes in physical function and health status resulting from injury disease or other causes. Demonstrates productive and positive Team collaboration for maximizing functional outcomes and care.
Required Minimum Knowledge Skills and Abilities (KSAs):

Education: EntryLevel Degree in Physical Therapy
License/Certifications: State of Maine licensure as a Physical Therapist. Current valid driver s license to operate a motor vehicle with vehicle available for work covered by liability insurance as specified by agency policy. Current certification in Basic Life Support for Healthcare providers.
Experience: Minimum one year of fulltime equivalent experience in a supervised setting.

Essential Functions
Physical Therapists are responsible for all aspects of patient/resident management including Examination Evaluation Diagnosis Intervention plans and Outcomes.
Reviews and determines appropriateness of each referral for PT services.
Accurately selects appropriate examination tools and procedures to determine current level of function i.e. ROM MMT Functional mobility interview observation and collaboration with other healthcare providers.
Evaluates data gathered during the examination and makes clinical judgments.
Refers patients/clients to other healthcare providers for services beyond the scope of physical therapy.
Establishes a physical therapy diagnosis (not a medical diagnosis).
Establishes a physical therapy prognosis establishes short term and longterm goals and a plan of care including treatment frequency and duration to achieve goals.
Selects appropriate interventions and modifies interventions as determined by patient/resident response and results of reexaminations. Interventions may include but are not limited to hot/cold packs paraffin ultrasound phonophoresis electrical stimulation massage myofascial release muscle energy strain/counterstrain neuromuscular reeducation therex/group therex transfer training gait training postural training assistive device selection and fitting seating and positioning CPM s iontophoresis taping patient/client/family education and home programs.
Monitors outcomes including the impact of physical therapy interventions in the following domains: pathology/pathophysiology impairments functional abilities and limitations disabilities risk reduction/prevention health/wellness/fitness Discharge planning community resources and patient/resident satisfaction.
Discharges patients from physical therapy when anticipated goals and outcomes are achieved in conjunction with team planning.
Discontinues patients from physical therapy when the patient/resident will no longer benefit from services financial/insurance resources have been expended medical status does not allow continuation of services or when the patient/client declines to continue with services.
Physical Therapists provide documentation of patient/client management.
Documentation is accurate complete and timely.
Documentation is completed according to SHC and department policy.
Completes all appropriate certification forms and thirdparty payer requirements in a timely manner.
Examples of documentation include but are not limited to daily attendance refusals initial evaluations discharge summaries daily & weekly progress notes insurance reviews home discharge instructions and home programs
Physical Therapists supervise Physical Therapist Assistants Rehabilitation Aides and Physical Therapy students.
Follows direction and supervision guidelines of the American Physical Therapy Association and rules as set forth by the Maine Physical Therapist State Licensure requirements.
Familiarizes self with job description of Physical Therapist Assistants and Rehabilitation Aides
Familiarizes self with the clinical student program.
Collaborates with the Physical Therapist Assistants to provide selected interventions.
Utilizes Physical Therapy Aides to assist with set up clean up and to assist with the provision of selected interventions.
Contributes to annual performance appraisal of Physical Therapist Assistants and Rehabilitation Aides.
Trains and evaluates physical therapy students according to the clinical student program and school requirements.
Physical Therapists maintain ongoing communication and collaboration with treatment team members.
Shares pertinent information with all necessary team members for collaboration toward maximal patient outcomes.
Contributes thoroughly in all applicable interdisciplinary meetings care planning discharge planning meetings patient care conferences or other meetings
Establishes dialogue with outside resources i.e. MCH CHANS Rehab facilities case managers utilization review and thirdparty payers.
Collaborates positively with Rehab Manager for meeting overall care needs of patients/residents.
Physical Therapists maintain a clean organized and hazard free work environment.
Remains knowledgeable of all safety infection control and fire/disaster policies and procedures.
Remains alert to any adverse reactions that patients may experience during physical therapy services.
Keeps patient areas free of obstacles.
Returns and cleans all patient equipment to designated areas for the convenience of other staff members.
Notifies aide of shortage in departmental supplies/inventory.
Notifies Rehab Manager and Facilities Management if there is any mechanical failure with department equipment
Physical Therapists participate in the education of departmental employees and the community to the role and function of physical therapy.
Provides inservices to departmental staff on continuing education topics.
Provides community education programs.
Participates in the departmental QA Program.
Demonstrates interest and value in continuing education and lifelong learning.
Sets annual professional goals and objectives and keeps supervisor informed of progress toward goals.
Completes mandatory education training by established deadlines.
Physical Therapists demonstrate age specific competencies for the populations served as documented in the department s scope of care policy.
