Malpractice Must be included in rates.

Orientation $250-262.50 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly $500-$525 - Per hour not OT

Holiday $750-787.50 - Must be 1.5x the hourly rate. Per holiday hour no OT

Nightly Call $Per night call to hold the pager to include 2.5 hours gratis

Holiday Nightly Call - $Per night call to hold the pager to include 2.5 hours gratis

On Call (Daily) $Per 24 hours to include 4 hours gratis

24 Hour Holiday Call Rate - Per 24 hours to include 4 hours gratis

Call Back $500-$525 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Schedule/Availability Requirements:

Availability: ASAP July and ongoing
Schedule - needed weekly:

• Friday: 5p - 7a (site willing to use provider for day shift on Friday if provider is available to start earlier)
• Saturday: 7a - 7a
• Sunday: 7a - 7a

Facility Location: Mercy Council Bluffs
Setting: Hospital setting performing 3600 IP/OP surgeries per year.
Duties: Independent Cases
FTE: 1.0
Required Procedures: Ortho Labor & Delivery Spine GI Gynecology Urology Cath Lab Interventional Radiology bariatric bypass minimal vascular cases
EMR: EPIC
Support Staff: Anesthesia providers CRNA Anesthesia Tech

Requirements: Iowa License Preferred (will consider provider with IMLC or willing to obtain IA license)
Credentialing with other facilities No (unless provider is willing to also cover at Immanuel in Omaha NE)
Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less
