Job Summary:

Obtains information for birth and death certificates reporting and prepares the certificate in accordance with state Vital Statistics rules and regulations. Collects statistics for reporting e.g. mortality.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Identifies and abstracts information regarding births/deaths to assist in completion of reporting to the State Department of Vital Statistics.
  • Prepares and electronically submits required reporting information to the States database.
  • Answers phone calls and welcomes visitors inquiring about birth certificate processing while observing confidentiality and providing information to all patients following departmental Health Insurance Portability and Accountability Act (HIPAA) and regulatory requirements in order to obtain optimal patient and customer satisfaction.
  • Performs other duties as assigned.


EDUCATION

  • High School diploma/GED or 10 years of work experience


EXPERIENCE

  • Two years experience

PHYSICAL REQUIREMENTS

  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting standing walking reaching and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms malfunctioning machinery etc.
  • Frequent keyboard use/data entry.
  • Occasional bending stooping kneeling squatting twisting and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Rare climbing.
