Job Overview

We are seeking a full-time Physical Therapist to join an outpatient neuro rehabilitation team in Medford Oregon. This role is ideal for a PT who enjoys helping patients improve mobility independence and overall function through individualized evaluation and treatment.

The Physical Therapist will work with patients with a variety of diagnoses including neurological and physical conditions while providing patient-centered care in an outpatient setting.

Key Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy services based on each patients functional level goals and needs.
• Support patients with neurological and mobility-related conditions.
• Help patients improve strength movement balance independence and quality of life.
• Collaborate with physicians care teams patients and families.
• Document treatment plans progress and outcomes accurately.
• Provide education and recommendations to support continued recovery and independence.

Qualifications

Required:

• Bachelors Masters or Doctorate degree from an accredited Physical Therapy program.
• Active Physical Therapist license in Oregon or Washington upon hire.
• BLS certification through the American Heart Association upon hire.
• Ability to provide care in all states where services are delivered.
• Strong patient care communication and documentation skills.

Preferred:

• Outpatient neuro experience.
• Specialized physical therapy training or experience related to department needs.
