The Salt River Pima-Maricopa Indian Community (SRPMIC) is a sovereign tribe located in the metropolitan Phoenix area. Bounded by the cities of Scottsdale Tempe Mesa and Fountain Hills the Community serves over 9000 tribal members and all people conduct business over approximately 96 square miles of residential agricultural and commercial lands as a full-service government entity.

What does the job look like
The Salt River Pima-Maricopa Indian Community (SRPMIC) Behavioral Health Reentry Team is seeking a dedicated Case Manager to support individuals returning to the community after incarceration.
This position focuses on providing intensive case management to clients who were recently incarcerated requiring someone who is comfortable working within Department of Corrections settings and collaborating closely with correctional staff.
In this role the Case Manager will conduct assessments develop service plans coordinate behavioral health and social services needs and referrals and maintain consistent contact with clients both in correctional facilities and in the community to ensure continuity of care.
The position also involves partnering with SRPMIC departments probation and parole and community organizations to support clients in accessing housing employment cultural resources and behavioral health services.
What do you have
The ideal candidate will have demonstrated experience providing case management to justice involved individuals along with experience working within a tribal community and a strong understanding of SRPMIC values traditions and cultural practices.
Candidates should possess strong communication and problem-solving skills the ability to build trust with individuals who may have experienced trauma or systemic barriers and a commitment to culturally grounded trauma informed care.
What do you need- Bachelors degree in counseling Social Work Psychology or closely related area.
- Experience equivalent to three (3) years of full-time in social services field or with community outreach work.
OR- Associates degree in a related field.
- Experience equivalent to five (5) years of full-time professional level experience in social services community outreach or a rehabilitation treatment setting will be acceptable.
- Experience with Tribal or Indigenous communities strongly valued.

Special Requirements
- May be required to work beyond normal work hours including nights weekends and holidays.
- Must have current certification in First Aid and CPR prior to working with patients.
- May be required to travel.
- Must possess and maintain a Valid Arizona Drivers License.
- Ensures all patient information is kept confidential and complies with Health Insurance Portability and Accountability Act (HIPAA) regulations and SRPMIC HHS policies and procedures.
