Position Overview
We are seeking a high-energy detail-oriented Intake Coordinator to join our home health team in Stratford. This is a critical role that serves as the bridge between our referral sources clinicians and patients. If you have a background in medical data entry and a passion for helping patients receive the care they need we want to hear from you.
Key Responsibilities
Heavy Data Entry: Accurately input patient demographics physician orders and insurance information into our electronic health record (EHR) system.
Referral Management: Process incoming referrals from hospitals rehab centers and physician offices efficiently to ensure timely starts of care.
Coordination: Perform basic intake functions including verifying insurance eligibility and obtaining initial authorizations.
Communication: Serve as a professional point of contact for patients families and clinical staff to gather missing documentation or clarify intake details.
Compliance: Maintain strict confidentiality and adhere to all HIPAA regulations regarding patient information.
On-Call Duty: 1 week every 4 weeks. After hours and weekends. On call is still active on major holidays. Starts after probationary period.
Requirements
Experience: Must have clinical in-field experience (e.g. CNA MA or Home Health Aide) AND/OR prior administrative/data entry experience within a medical or home health setting.
Skills: Fast and accurate data entry skills are a must.
Knowledge: Familiarity with medical terminology and home health intake processes.
Local: Ability to work on-site at our Stratford CT office daily.
Attributes: Strong organizational skills and the ability to stay calm under pressure in a high-volume environment.
Competitive hourly pay ($20$22/hr based on experience).
