Job Summary:
The Staff Pharmacist - Meridian provides clinical pharmacy services to the company and consumers through the appropriate evaluation of physician orders compounding of sterile preparations and safe and efficient dispensing of medications supplies and administration equipment.
Duties/Responsibilities:
- Compounds sterile preparations for dispensing using aseptic technique.
- Maintains knowledge of USP chapter <797> to assure the quality sterility and stability of compounded sterile preparations.
- Uses available resources to gather information on drugs.
- Assists in the maintenance and cleaning of pharmacy work areas in accordance with USP Chapter <797.
- Acts as a resource and consultant to prescribers.
- Acts as a resource and consultant to prescribers referral sources and consumers regarding the proper use of medications administration supplies and administration equipment.
- Acts as the patient care coordinator with outside nursing agencies; coordinates with nursing personnel regarding patient care drug delivery and clinical assessment.
- Assesses prescription drug orders for accuracy appropriate dosage and usage and drug interactions.
- Evaluates patients for medication adherence and addresses barriers to increased compliance.
- Stays knowledgeable of current clinical issues industry practice standards and accreditation standards and incorporates them into daily practice.
- Maintains appropriate documentation of the care delivered to consumers and of drug prescriptions and compounding as described in the Apollo Policy and Procedure Manual and as mandated by individual State Board of Pharmacy requirements.
- Completes assigned paperwork/document as directed in a timely manner.
- Assists in other departments as needed.
Required Skills/Abilities:
- Assists in other departments as needed.
- Good interactive skills with employees referral sources Physicians pharmaceutical reps and consumers.
Education and Experience:
- Actively licensed pharmacist in good standing with the Board of Pharmacy.
- Minimum three years of clinical experience.
- Minimum one year experience with infusion therapy and sterile compounding.
- Sterile Compounding training.
