Job Summary
Job Title: RN Case Manager / Utilization Review / CDI
Location: Syracuse NY
Contract Duration: 14 Weeks
Pay Rate:
Local: Up to $56/hr
Travel: Up to $65/hr
Shift & Schedule: Monday Friday Weekly Hours: 40 Hours
Position Overview:
Navitas Healthcare LLC is seeking RN Case Manager / Utilization Review / CDI for an exciting Travel or Local job in Syracuse NY.
Responsibilities:
- Coordinate and manage patient care plans throughout hospitalization
- Perform utilization review to ensure appropriate level of care and resource use
- Collaborate with physicians nurses and interdisciplinary teams for discharge planning
- Review clinical documentation for accuracy completeness and compliance
- Identify CDI opportunities and communicate recommendations to providers
- Monitor patient progress and assist with transitions of care
- Document accurately in EPIC EMR
- Communicate with patients families insurance providers and care teams
- Ensure compliance with hospital policies and payer requirements
- Participate in case reviews audits and care coordination meetings
Qualifications:
- Active RN license (NY)
- BLS required
- 1.5 years acute care experience
- Acute care case management experience required
- EPIC EMR experience required
- Strong knowledge of utilization review and CDI processes
