Job Summary
ICCW Intensive Care Coordinators with Wraparound (previously known as Wraparound Coordinators)
The ICCW Coordinator is responsible for developing and appropriately executing comprehensive and outcome-based treatment plans completing related casework and referring out to additional services to the clients and families they assign within a community setting.
RESPONSIBILITIES:
- Casework:
- Initiate contact with assigned clients and families and arrange for first session within contractually indicated time parameters.
- Maintain regular in person sessions with assigned clients families and their system partners following program intent and the stages of wraparound.
- Timely completion of all required paperwork in accordance with programs strengths and outcome-based guidelines based upon a variety of presenting conditions.
- Maintain appropriate and timely progress notes on each client.
- Assist family teams with coordination of community-based resources classes groups and/or services for identified needs including the development of natural supports.
- Monitor the quality of overall services to families on his/her caseload and evaluate the adequacy of these services to meet treatment outcomes through established best practices.
- Maintain up to date with community mental health provided services and complete necessary paperwork in a timely manner.
- Provide transition and discharge planning that coordinates community and family resources to promote continued growth following program completion.
- Professional Skills:
- Meet bi-weekly with supervisor through direct supervision for case review and the mutual sharing of feedback on cases review and professional growth.
- Participate regularly in bi- monthly team supervision where program updates are shared and a case review is done as a team.
- Complete and document all required agency and state required trainings for continued development.
- Maintain regular office hours and presence within Bouma Integrated programs that is mutually agreed upon with the wraparound supervisor.
- Maintain in communication with system partners outside of wraparound meetings and document contacts.
- Respect others and understand their needs and feelings regardless of race social ethnic economic and religious background; have an attitude and aptitude for providing nurturance being patient and accepting others.
- Responsible for timely Quality Assurance compliance as related to employees roles and functions.
- Responsible for quality assurance compliance including but not limited to participating in peer reviews executing plans of correction and collecting service satisfaction data.
- Represents Wedgwood Christian Services within Marketing and Public Relations requested activities.
- Other duties as assigned.
QUALIFICATIONS:
- Bachelors degree in social work Psychology or another associated human service profession clinical/mental health experience required.
- 2-3 years of experience working with children and/or families in the Human Services industry with an interdisciplinary team preferred.
- Prior experience working in a community based or in-home service area preferred.
- Extensive working knowledge of community resources (Kent County) preferred
- For transportation purposes must have a valid drivers license and reliable transportation.
- Subscription to the Statement of Faith of Wedgwood Christian Services and agreement with the goal purpose and objectives of the agency.
HOURS OF WORK: Full-time 40 hours per week some evening availability required.
