Job Title: Care Mobilisation Coordinator

Location: London Bridge London SE1 UK (Hybrid 1 day home 4 days office)

Salary: Up to 32000 depending on experience

Contract: Full Time Permanent 37.5 hrs a week some on-call rota responsibilities

In this key role youll coordinate the onboarding of new complex care packages liaising with clients families and internal teams to ensure seamless care delivery. About the Complex Care Mobilisation Coordinator Role:

As a Complex Care Mobilisation Coordinator youll work closely with our Clinical Recruitment and Coordination teams to mobilise new care packages efficiently and effectively. This involves scheduling assessments assigning care staff preparing rotas and maintaining compliance records. Youll be the key point of contact for clients families and commissioners providing updates and escalating issues as needed.

Key Responsibilities of the Complex Care Mobilisation Coordinator

Coordinate new complex care package mobilisation under senior guidance
Communicate effectively with clients families commissioners and internal teams
Schedule client assessments in partnership with the Clinical Team
Work with Recruitment and Coordination to assign care staff
Create staff profiles arrange introductions and prepare initial rotas
Maintain mobilisation records in care management systems
Ensure compliance with CQC guidelines and company policies
Provide updates to stakeholders escalating issues as needed
Hand over care packages smoothly to the long-term care team

Skills and Experience Required for the Complex Care Mobilisation Coordinator

Experience in care coordination administration or mobilisation
Knowledge of complex care and CQC standards
Strong communication and relationship-building skills
Excellent time management and attention to detail
Proficiency in Microsoft Office and rostering systems
NVQ Level 2 in Health & Social Care (or equivalent)
