Job Summary

JOB TITLE: TLS Case Manager

REPORTS TO: Field Based Housing Supervisor

SCHEDULE: Monday-Friday (9:00am-5:00pm)

LOCATION: Los Angeles CA / Venice CA / Santa Monica CA

STATUS: Full-Time 40 hours/week Non-Exempt

SALARY RANGE: $24.00-$25.00/hr

Safe Place for Youth (SPY) is the leading service provider for youth experiencing or at-risk of homelessness in West Los Angeles. Founded in 2011 our agency offers trauma-informed low barrier programs that support young people in exiting the streets and achieving stability. Our robust continuum of care includes street outreach drop-in services health and wellness programming case management education and employment services and housing programs.

ABOUT THE ROLE:

Time-limited Subsidy is a Housing First program focused on providing solutions to unhoused youth by assisting them to move into permanent housing. With the assistance of the Case Manager transitional aged youth receive services and resources to build life skills that support youth to meet their responsibilities.


WHAT YOULL DO:

  • Support and coach youth to complete housing applications accompany them to leasing appointments and other support associated with the housing placement process.
  • Conduct housing searches and identify housing options suitable for youth and in compliance with program requirements.
  • Provide case management to connect youth to wraparound supports through SPY and other community resources.
  • Ongoing assessment of youth needs and developing individualized housing and service plans that are updated monthly in partnership with youth.
  • Provide crisis management and de-escalation interventions as needed (e.g. minimize risk that could jeopardize the tenant-landlord relationship).
  • Engage youth in life skills workshops and activities (e.g. how to budget utilizing public assistance benefits accessing free or low-cost services conflict resolution etc).
  • Engage youth in increasing their income to ensure they are able to sustain market rate housing.
  • Refer and link youth to community services such as education and employment opportunities mental health resources legal aid etc.
  • Organize schedule and facilitate youth and team meetings.
  • Maintain youth files in compliance with program standards.
  • Submit documentation notes and reports in a timely manner.
  • Attend all required meetings and trainings.
  • Build and maintain relationships with landlords and community agencies.
  • Other duties as assigned to support the mission of the organization
  • Advocating for youth while increasing landlord engagement.
  • Collaborate with various teams to assist members in meeting goals.
  • Meet with members in-person.
Requirements

WHAT YOULL NEED:

  • B.A. or B.S. in Psychology Sociology or related field preferred.
  • 1-2 years of experience in case management counseling or related experience is required.
  • 1-2 years of experience working with individuals experiencing or at risk of homelessness is preferred.
  • Experience working with youth preferred.
  • Individuals with lived experience of homelessness incarceration or addictions are strongly encouraged to apply.
  • Valid California drivers license personal vehicle and insurance
  • Familiarity with Los Angeles social services resources and the ability to creatively problem solve member issues.
  • Strong written and verbal skills
  • Proficiency with electronic record keeping systems and/or Clarity preferred.
  • High level of personal accountability for the quality and impact of work.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Changes in the environment such as the office or outdoors.
  • Physical effort/lifting such as sedentary- up to 15-20 pounds at times.

SPECIAL CONDITIONS:

Mandated Reporting Requirements: All SPY employees are mandated reporters and are legally required to report any reasonable suspicion of child elder or dependent adult abuse or neglect.

Driver Positions:

Applicants are required to provide a copy of their driving record from the Department of Motor Vehicles along with a copy of their valid drivers license and proof of insurance. Job offers are contingent upon a satisfactory driving record.
