As a LPN/LVN Clinical Coordinator youll contribute to our success in the following ways:
- Reviews and clears HCHB coordination notes and processes administrative task workflows in an accurate and timely manner.
- Obtains orders from physicians including verbal written and faxed orders and enters into appropriate system for CTM review and approval.
- Monitors patient hospitalization list confirms status and coordinates appropriate resumption of care.
- Discharges patients in inpatient facility at end of episode (EOE)
- Obtains patient-related attachment renewal.
- Participate in High-Risk Meetings and ensure that the best provision of services is offered.
- Available to provide direct care as needed.
- Answers telephone calls from patients patient representatives and medical professionals and communicate messages to the appropriate party or parties.
- Provides clerical support to patients such as but not limited to scheduling follow-up appointments for patients (as requested) providing health summaries to case managers obtaining pre-authorizations and addressing patient questions or concerns.
What is Required
- Graduate of an accredited school of professional nursing
- Current Certified Licensed Vocational Nurse or Licensed Practical Nurse
- 1 year of home care or hospice experience
- Able to seek patient feedback advocate for patients understanding of patient needs and managing patient expectations
- Able and willing to travel up to 50% as needed to support business and patient needs
- Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
- Valid drivers license and insurance and reliable transportation to perform job tasks
You will report to the Clinical Team Manager.
