What Youll Do

As a PRN PT serving our Elkhorn senior care community you will:

• Complete functional mobility and physical performance assessments

• Create individualized treatment plans tailored to resident needs

• Provide skilled PT interventions that improve mobility strength and quality of life

• Educate residents families and caregivers on fall prevention and safe mobility

• Recommend assistive devices and functional adaptations

• Collaborate with an interdisciplinary team to support whole-person care

• Maintain accurate clinical documentation

Qualifications

• Graduate of an accredited PT program (Bachelors Masters or Doctorate)

• Licensed or eligible for Wisconsin PT licensure

• Strong clinical judgment communication skills and professionalism

• A passion for improving the lives of older adults
