You will provide person-centred support to individuals with learning disabilities autism mental health needs or health conditions across different care settings.
Shift Patterns
This role may includea mix of:
Day shifts
Late shifts
Sleep-in shifts
Waking night shifts
Every other weekend working
Flexibility is essential to meet the needs of the service.
Key Responsibilities
Delivering high-quality support
Assisting with personal care daily living tasks and health needs
Promoting independence choice and dignity
Supporting individuals to access the community and achieve personal goals
Following care plans risk assessments and clinical guidance
Working collaboratively as part of a new team
