Description:
ESSENTIAL FUNCTIONS
Knows understandsincorporatesanddemonstratesthe missionvisionand values of Trinity Health in leadership behaviorspracticesand decisions.
Assesses plans evaluates and coordinates management of patient care including discharge planning andutilizationneeds.Supportsministryand system goals for capacity management through achieving length of stay goals identification of avoidable delays at department approved levels and other Case Management initiatives.
Coordinateswiththeinterprofessionalteam in meetingpatient caregoals.
Establishes partnerships with sitePatient Experiencechampionstoalignefforts with patient satisfaction outcomes.
Identifiesdischarge needsdeterminestheappropriate levelof care that the patient can transition tocoordinates planning and problem solves continuity issues to ensure seamless continuum of patient careand prevent the patient from having to readmit to the hospital.
Ensuresallappropriate clinicalinformation is provided to outside agencies.
Communicates pertinent informationtotheappropriate membersof the patient care teamboth effectively and professionally.
Identifiesandeliminatesbarriers to effective patient stay management and promotes improvements in work processes within department hospital and community.
Pursues professionaldevelopmentopportunitiesandmaintainslicensure.
Advocates for patient legal social and ethical rights while balancing patient resources and hospital financial stewardship.
Refers quality issuesand poorutilizationof servicestotheappropriate sources.
Coordinates/facilitatespatient/family education.
Educates hospital leadership andcolleaguesto current case management practices patient coordination issues and unit specific trends.
Provides consultation to professionalcolleaguesand employeesregardingpatient stay options financial considerations psychosocial concerns risk management and ethical issues.
Documentspertinent assessments interventionstransitionsandoutcomes for patientsin accordance withdocumentation standards policiesproceduresand/or guidelines.
Utilizescomputer systems.
Maintains a working knowledge of applicable Federal State and local laws and regulations Trinity Health Integrity and Compliance Program and Code of Conduct as well as other policies and procedures to ensure adherence in a manner that reflects honest ethical and professional behavior.
MINIMUM QUALIFICATIONS
Valid license as an RN.BSN RequiredMasterspreferred.
BLS Certification Required.
3-5 years Case Management experience hospital or community based.

Possesses knowledge of budget processes quality improvement report distribution TheJointCommissionstandards stateand federal regulations six sigma and LEAN methodologies.
Proficient in common software applicationsand electronic medical records.(Word Excel Power point).
PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
Mustbe able totravel.
Must be able to adapt tofrequentlychanging work priorities.
Must be able to set and organize own workpriorities andadapt said priorities as they changefrequently.
Manual dexterity is needed tooperatea keyboard.Hearing is needed for extensive telephone and in person communication.
Pay Range: $67.00 - $70.00 (based on assignment option and shift). Pay is based on experience skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
