High Intensity Support Worker

Sublime Care is looking for experienced support workers for our complex clients in a range of different settings.

AINs OT Nursing or Med science students or experienced DSWs with a clinical edge: this role was built for you. Casual shifts across days evenings weekends and overnights mean you can fit work around study or other commitments without compromising on the quality of what you do.

• Have hands-on experience with personal care complex support needs or clinical settings
• Want variety: different participants different settings different challenges
• Can follow clinical support plans with precision and communicate clearly
• Want to be upskilled by experienced Registered nurses- paid for training onsite and in house

What Youll Be Doing

Youll deliver person-centred support in participants homes SIL/ SDA and in the community from daily living and personal care through to skill development and community participation. Youll implement individualised support plans document accurately and escalate risks when needed.

High Intensity Supports (with in house training): Complex manual handling Bowel and catheter care Enteral feeding / PEG support Dysphagia and mealtime management Diabetes management Seizure management Subcutaneous injections

Youll Need:

• NDIS Worker Screening Check
• Working With Children Check
• Driver Licence comprehensively insured vehicle
• First Aid Certificate (HLTAID011)
• COVID-19 vaccination (minimum two doses)
