To provide a full range of social work services to patients and their families who are admitted to Complex Malignant Heamatology. Role responsibilities include psychosocial assessments counselling locating and arranging resources discharge planning/coordinating linkage to community resources facilitating Advance Care Planning Conversations and addressing complex care issues and participating as a key member of the patient care team.
Qualifications/Skills
Masters Degree in Social Work required
Member in good standing of the Ontario College of SW and SSW
Knowledge of Canadian Social Work Competencies in hospice palliative care an asset
Experience with grief and end of life care an asset
Minimum 3 years of experience working as an MSW in a Hospital; experience working with Complex Malignant Haematology patients and families required
Commitment to Person Centred Care Best Practices
Excellent communication organization skills and time management skills
Strong teambuilding skills and conflict resolution skills
Excellent assessment interviewing and problem solving skills
Knowledge of the health care system and community resources
Superior ability to manage multiple tasks and set priorities in a fast-paced environment
Knowledge of and past clinical experience in Inter Professional Collaboration and Inter Professional Education
Commitment to student education
Interest in exploring Academic Practice opportunities that address Patient and Family Care across the continuum of Health Care
