Summary of Duties
To provide a full range of social work services to patients and their families who are admitted to Complex Malignant Heamatology. Role responsibilities include psychosocial assessments counselling locating and arranging resources discharge planning/coordinating linkage to community resources facilitating Advance Care Planning Conversations and addressing complex care issues and participating as a key member of the patient care team.
Qualifications/Skills
- Masters Degree in Social Work required
- Member in good standing of the Ontario College of SW and SSW
- Knowledge of Canadian Social Work Competencies in hospice palliative care an asset
- Experience with grief and end of life care an asset
- Minimum 3 years of experience working as an MSW in a Hospital; experience working with Complex Malignant Haematology patients and families required
- Commitment to Person Centred Care Best Practices
- Excellent communication organization skills and time management skills
- Strong teambuilding skills and conflict resolution skills
- Excellent assessment interviewing and problem solving skills
- Knowledge of the health care system and community resources
- Superior ability to manage multiple tasks and set priorities in a fast-paced environment
- Knowledge of and past clinical experience in Inter Professional Collaboration and Inter Professional Education
- Commitment to student education
- Interest in exploring Academic Practice opportunities that address Patient and Family Care across the continuum of Health Care
