YOUR NEW ROLE:

Holstep Health Medical - West Heidelberg is seeking a dynamic and system-oriented Practice Nurse with a strong interest in primary health. This diverse role requires sound clinical skills and the capacity to develop a leadership role within the GP team.

The successful applicant would be part of a team assisting GPs with the management of clients with complex needs including mental health and addiction issues. Responsibilities will include planning and coordinating care case conferencing some management and staff support.

YOU WILL BE RESPONSIBLE FOR:

This role is responsible for the provision of high-quality nursing care in a community-based General Practice setting in collaboration with the GPs. The role will offer support vision and leadership to develop practice efficiencies. This role is also responsible to provide support to the Practice Manager and administrative staff as required.

The Practice Nurse role is essential in providing appropriate clinical support for Holstep Health GP services and to support the efficient and professional delivery of health care.

WHAT YOU NEED TO SUCCEED:

• Current Division 1 Registered Nurse registration.

• Minimum three years experience as a division 1 Registered Nurse.

• Demonstrated patient-focused experience in undertaking health assessments goal directed care plans and reviews.

• Demonstrated ability to work independently and as part of a wider multidisciplinary team and health professionals.

• Commitment to consumer directed care and optimal client care through a high level of clinical expertise.

• High level communication skills (verbal and written).

• BLS Certification.

• Demonstrated experience in relevant clinical areas including wound care Pathology Collection and immunisations.
