Scheduled Weekly Hours:

Emplify Health by Gundersen is seeking a RN Care Coordinator for our Hospice this role you will work 12-hour day shifts every Friday Saturday- work 72 hours and get paid for 96!

What you will do:

• 0.9 FTE72 hours/every two weeks

• 12-hour day shifts 9am-9pm every Friday Saturday and Sunday

• Weekend package pay incentive (Employees will receive a Weekend Only premium equal to .3333 times base pay for the actual hours worked during these Friday Saturday and Sunday shifts; which is equivalent to 96 hours of pay for 72 hours worked.)

• Provide compassionate care to families and patients thru end-of-life transitions within a 50-mile radius of La Crosse WI

• Work with a multidisciplinary team consisting of nurses social workers chaplains bereavement coordinators and hospice aides

What you need:

• Associate Degree in Nursing

• Minimum of 1 year RN experience

• RN licensure to practice in the state of Wisconsin upon hire

• RN licensure to practice in the state of Minnesota within 3 months of hire
