Job Summary
Job Title: Licensed Practical Nurse (LPN) Long Term Care (LTC)
Location: Grand Junction CO 81506
Shift: Variable Day Shift (12-hour shifts)
Schedule: 3 days/week (36 hours guaranteed)
Duration:13 Weeks
Position Summary
A healthcare facility is seeking a dedicated Licensed Practical Nurse (LPN) with experience in Long Term Care (LTC) to support patient care in a skilled nursing environment. This role requires delivering high-quality care while maintaining compliance with healthcare regulations and standards in a fast-paced setting.
Key Responsibilities
- Provide direct patient care in a long-term care setting
- Monitor patient conditions and promptly report changes to the care team
- Administer medications and treatments as prescribed
- Maintain accurate and timely patient records and documentation
- Collaborate with physicians registered nurses and other healthcare staff
- Follow safety protocols and infection control guidelines
- Assist residents with activities of daily living (ADLs)
Qualifications
- Active LPN/LVN license required
- Previous experience in Long Term Care (LTC) preferred
- Strong clinical and patient care skills
- Ability to work 12-hour day shifts in a dynamic environment
- Excellent communication and teamwork abilities
