Responsibilities
At Change Grow Live you will be working with people to help them change the direction of their life grow as an individual and live life to its full potential.
Making the decision to enter treatment can be an intimidating experience so its paramount that we create a welcoming and caring environment which is non-judgmental.
Assessment is a key opportunity to engage people your role will be to listen to our service users to understand their needs and support them to access the right intervention at the right time in a way that best meets their needs.
You will advising service users about the options that are available to them both within the service and the wider community and offering them appropriate knowledge advice and equipment to enable them to keep themselves safe.
You will be provided with all the skills knowledge training and support to undertake the role.
Key Activities:
- Support the team in completing triage assessments for those entering services.
- Provide service users with information on the treatment/options available to them both face to face and via the telephone.
- Support service users to access treatment and support.
- Ensure the accurate and timely recording of client data and information in order that the service operates within its requirements.
Assist and carry out appropriate screening processes including urine saliva tests and Dry Blood Spot tests.
