We are seeking an Advanced Practice Provider to join one of the nations most integrated healthcare systems with over 1300 providers in Indianapolis IN. This advanced practice provider will be focused on the well-being of Onsite employees and their beneficiaries at AMPG (6925 South Harding Indianapolis IN 46217).
Highlights
Community Health Network has an excellent opportunity for an experienced family medicine certified advanced practice provider to practice in an Employer Health Onsite primary care role. Minimum two years of experience as an advanced practice provider is required.

Managing chronic or complex health conditions
