Job Summary
We are seeking a licensed Physical Therapist to provide inpatient acute care services across two healthcare facilities within the same regional health system. The therapist will deliver comprehensive rehabilitation services to patients recovering from illness injury or surgery focusing on restoring mobility improving functional independence and promoting optimal recovery outcomes.
This role involves a split schedule between two facilities typically working three days per week at one location and two days per week at another though the exact schedule may vary based on operational needs. One weekend day may be included periodically with flexibility in scheduling.
Key Responsibilities

Evaluate patients physical conditions and functional limitations to develop individualized treatment plans.

Provide therapeutic interventions aimed at restoring mobility strength balance and functional independence.

Collaborate with physicians nurses and other rehabilitation professionals to coordinate patient care.

Educate patients and family members on treatment plans exercises and strategies for continued recovery.

Monitor patient progress and modify treatment plans as necessary to achieve rehabilitation goals.

Maintain accurate and timely documentation within the electronic medical record system.

Ensure compliance with facility policies regulatory standards and professional practice guidelines.

Participate in interdisciplinary care planning and discharge planning as needed.
Qualifications

Current and active Physical Therapist license in the state of practice.

Minimum of one year of physical therapy experience preferably in an inpatient or acute care setting.

Graduation from an accredited Physical Therapy program.

Basic Life Support (BLS/BCLS) certification required.

Experience working with electronic medical record (EMR) systems preferably Epic is preferred.

Strong clinical assessment communication and patient care skills.
Additional Information

Work schedule includes 36 40 hours per week with flexibility.

Position involves travel between two facilities within the same region.
