We are looking for a dynamic Rehab Director Occupational Therapist-Occupational Therapist Assistant!

Greenfield Rehabilitation Agency is seeking an exceptional Occupational Therapist (OTR) or Certified Occupational Therapy Assistant (COTA) to serve as Rehab Director at our senior living community and skilled nursing facility in Galesville Wisconsin.

As Rehab Director You Will:
Lead and support an interdisciplinary therapy team
Build and sustain a resident-centered therapy culture
Mentor clinicians to deliver high-quality evidence-based care
Drive clinical excellence compliance and operational efficiency
Foster strong relationships with residents families and facility partners
Oversee scheduling staffing productivity and documentation standards
Ensure regulatory readiness and adherence to state and federal guidelines
Promote a positive accountable team-oriented environment

Qualifications
Licensed OTR or COTA in the state of Wisconsin
Leadership or supervisory experience in senior care or SNF preferred
Strong communication organization and team-building skills
Passion for serving the geriatric population
