Roles & Responsibilities:
CASEWORK AND COUNSELLING
Manage 25-30 C&C active cases (mainly low-moderatelevel of complexity) per quarter.
Conduct assessmentplanningand intervention for cases bytaking into accountthe integration of a range of information including practice guides and organisational frameworks.
Conduct case conferences with partners.
Participatein networking and case management meetings.
Document and evaluate case outcomes.
Develop and reviewappropriate interventionstrategies.
Ensure proper documentation of cases.
Ensure reports on cases aresubmittedin a timely manner.
INFORMATION AND TRIAGE
Manage case referrals.
Part of the Intake & Triage Team to conduct intake assessments for referred clients based on the practice standards of the Code of Social Work Practice and Practice Guide.
Refer clients toappropriate agenciesfor further intervention.
COMMUNITY WORK
Network with other organizations and stakeholders.
Participatein implementing and evaluating of community outreach/ initiatives.
GROUP WORK
Be part of the programme team for Group Work conducted at the FSC.
REQUIREMENTS
Degree or Graduate Diploma in Social Workor related field such asPsychology and other relevant fields in Social Science
Previousexperiences via internships or placements in related areas of work will be considered
Understanding of social work values and ethics
Understanding of work with vulnerable populations i.e. low-income families
