Associate (Social Work) FSC Punggol

INFORMATION AND TRIAGE

Manage case referrals.

Conduct intake assessments for referred clients based on the practice standards of the Code of Social Work Practice and Practice Guide.

Refer clients to appropriate agencies for further intervention.

Ensure proper documentation of referrals and intakes.

Ensure reports are submitted in a timely manner.

COMMUNITY WORK & GROUP WORK

Assist in implementation of community outreach / initiatives.

Assist in implementation of groupwork sessions.

Assist in administrative needs.

ADMIN

Assist in facility management related functions.

Assist in data management and reporting.

Assist in other administrative functions tasked by Centre Management Team (CMT).
