Position Overview
A leading acute care hospital in South Florida is seeking an experienced Director of Case Management to lead and oversee daily case management operations. This leadership role is responsible for care coordination discharge planning departmental staffing and ensuring compliance with regulatory and organizational standards.
The Director will collaborate closely with hospital leadership and interdisciplinary teams to improve patient outcomes ensure efficient resource utilization and support continuity of care.
Key Responsibilities
Direct and oversee daily case management operations including care coordination and discharge planning.
Manage department staffing onboarding competencies and skill mix to meet operational and quality objectives.
Allocate resources effectively to maintain productivity and performance standards.
Ensure compliance with organizational policies regulatory requirements and care coordination protocols.
Collaborate with senior hospital leadership including finance medical nursing and case management executives.
Monitor departmental performance through data analysis reporting and performance improvement initiatives.
Lead interdisciplinary teams to ensure efficient patient care coordination and timely discharge planning.
Establish strong working relationships with internal departments and external healthcare agencies to support continuity of care.
Promote a culture aligned with the organizations mission values and ethical standards.
Required Qualifications
Bachelors Degree in Nursing Social Work Healthcare Administration or a related field.
Active Registered Nurse (RN) license OR
Active Licensed Clinical Social Worker (LCSW) OR
Active Licensed Master Social Worker (LMSW).
3 years of case management experience in an acute care hospital setting.
Recent experience working in an acute care hospital environment.
