The facility is seeking a Nurse Practitioner House Call to join its expanding care team in Brooklyn New York. This is a full-time opportunity. The role focuses on delivering high-quality patient-centered care by conducting in-home medical visits for patients who may have limited access to traditional clinic settings.
Provide comprehensive in-home medical assessments including patient history and physical examinations.
Diagnose and manage acute and chronic medical conditions.
Develop and implement individualized treatment plans based on patient needs.
Order and interpret diagnostic tests including laboratory work and other relevant studies.
Provide medication management and monitor patient response to treatment.
Educate patients and caregivers on disease management preventive care and medication adherence.
Coordinate care with physicians specialists home health agencies and family members.
Document patient encounters accurately and promptly within the electronic health record (EHR).
Participate in care coordination and quality improvement initiatives.
Masters or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
Board Certification as a Nurse Practitioner (FNP ANP or AGNP) through ANCC or AANP.
Active or eligible Nurse Practitioner license in New York.
Active Registered Nurse (RN) license in New York.
DEA registration (or eligibility).
Experience in primary care geriatrics home health or internal medicine preferred.
