Job Summary
Job Summary (List Format) Home Health & Hospice RN
- Patient Population: Home Health and Hospice patients (CHF Diabetes Renal Disease wound care IV therapy management of drains/devices)
- Required Certifications: BLS
- Experience Requirements:
- Recent Home Health experience (within last 2 years)
- Proficiency in OASIS documentation
- Wound care experience
- Excellent infection control practices in the home
- Patient Load: Typically 6 patients per day (Admission 3-4 patients)
- Key Duties:
- Conduct Start of Care (SOC) Re-certifications and Resumptions
- Provide care for both Home Health and some Hospice patients (primarily Home Health)
- Staff Support: Access to Administrator RN coordinator and office support supervisor after hours
- CNA Support: CNAs available for assistance
- Schedule:
- 5 days per week (Mon Fri 8:00 AM 4:30 PM)
- Rotating weekends (every 4th or 5th weekend with compensatory days off)
- Evening/night on-call rotation (typically one night/week)
- Holiday Requirement: Work up to 2 holidays per year
- Dress Code: Any solid color scrubs clean and professional
- Other Requirements:
- Must clock in/out at facility
- Previous Marshfield employees must have a 6-month gap before reapplying
- Must note in submission if previously worked at Marshfield
- Additional Notes:
- No ventilator support provided
- No utilization review duties
- On-call rotation required may see hospice patients during call (protocols provided)
- Local rate reduction applies for candidates within 80 miles of facility
