The Case Manager will play a crucial role in supporting the residents referred by HRA. Responsibilities include providing comprehensive case management services and counseling tailored to the needs of these individuals. The successful candidate for this position should demonstrate strong clinical skills and prior experience working with individuals diagnosed with co occurring disorders and/or trauma.
RESPONSIBILITIES: Maintain a caseload of the individuals diagnosed with serious mental illness and/or substance use disorder and provide supportive case management services to them Develop and facilitate therapeutic groups for residents Maintain resident records in compliance with agency and funding standards Conduct and review assessments Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self sufficiency Monitor and ensure residents are linked to appropriate treatment and services Provide counseling and crisis intervention as necessary Assist residents in securing and maintaining benefits Maintain a liaison with community services including hospitals substance abuse treatment programs HRA and mental health programs Monitor and assist residents with rent payments and budgeting Conduct regular unit inspections Educate residents about approaches to mental health and substance use Compile resources on mental health information and treatment strategies Attend ongoing trainings to enhance the services provided to the residents Assist with completion of reports as needed Other duties as assigned
QUALIFICATIONS: A Bachelors degree in Psychology Mental Health Counseling Social Work or a relevant field. Masters degree preferred. At least 3 years of experience working with homeless individuals diagnosed with serious mental illness and/or substance abuse disorder and/or in trauma settings Expertise in Evidence Based Practice including Critical Time Intervention Motivational Interviewing and Harm Reduction techniques Excellent clinical and case management skills including knowledge of benefits in the NYC Metro area Sensitivity to community issues and needs Outstanding interpersonal and communication skills both oral and written Strong ethical standards confidentiality and personal integrity Computer proficiency (i.e. email Microsoft Word Excel and PowerPoint
