**Core Responsibilities:**
The responsibilities encompass but are not limited to the following:
- Engage comprehensively in all stages of the Case Management Program (CMP) ensuring adherence to established case management (CM) standards of care.
- Provide advanced nursing insight into the CM process which includes assessment planning implementation coordination and monitoring. Identify and leverage opportunities for case management while integrating local case management processes.
- Formulate and execute local strategies employing a blend of inpatient outpatient onsite and telephonic case management approaches.
- Design and implement tools to facilitate effective case management such as patient identification and assessment tools clinical practice guidelines algorithms CM software and databases for community resources.
- Integrate case management (CM) with utilization management (UM) while aligning nursing case management with the practices of social work case management.
- Maintain collaborative relationships with community agencies and organizations to enhance resource utilization.
- Systematically collect and document patient care data with accuracy and integrity.
- Develop personalized treatment plans that incorporate preventive therapeutic rehabilitative psychosocial and clinical interventions ensuring continuity of care with the goal of achieving optimal wellness.
- Establish and implement mechanisms to assure the fidelity of patient treatment plans and effective follow-up post-discharge across ambulatory and community healthcare settings.
- Deliver targeted healthcare education to patients and/or caregivers based on assessed learning needs.
**Work Schedule:**
Monday to Friday requiring 9 hours of service between 7:00 AM and 4:30 PM inclusive of a one-hour lunch break.
**Minimum Qualifications:**

**Degree:** Associates degree in nursing.-
**Education:** Graduation from an accredited nursing program recognized by a regional or national accrediting agency such as the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).-
**Experience:** A minimum of 1 year of experience as a Registered Nurse Case Manager within the last 24 months.-
**Certification:** Must hold one of the following certifications:
- Certification from the Commission for Case Manager Certification (CCM)
- Certified Disability Management Specialist (CDMS) from the Certification of Disability Management Specialists Commission
- Certified Rehabilitation Registered Nurse (CRRN) from the Association of Rehabilitation Nurses
- Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S) from the American Board for Occupational Health Nurses
- Advanced Certification in Continuity of Care (ACCC) from the National Board for Certification in Continuity of Care
- Certified Rehabilitation Counselor (CRC) from the Commission on Rehabilitation Counselor Certification
- Nurse Case Manager certification (RN-NCM) from the American Nurses Credentialing Center
- Care Manager Certified (CMC) from the National Academy of Certified Care Managers

**Licensure:** An active unrestricted RN license from any state.-
**Life Support Certification:** Current Basic Life Support (BLS) Healthcare Provider certification from ARC or AHA.
- **Security:** Eligible to pass a government background check/security clearance.
