Your Role as a Baylor RN
Youll bring high-quality compassionate care directly into patients homeswhen they need it most.
Manage a weekend caseload within your local territory
Conduct skilled nursing visits assessments and follow-ups
Develop and adjust personalized care plans
Partner with physicians and care teams to ensure continuity of care
Educate patients and families to promote independence and safety at home
What Youll Need
Active RN license (CT)
1 year of nursing experience
Willingness to travel locally for patient visits
Valid drivers license reliable vehicle & insurance
Sit stand bend lift and move intermittently and be able to lift 50-100 lbs
Nice to have:
Home Health or Hospice experience
HomeCare HomeBase familiarity
