3 months contract with local authority
This role is designed for an experienced practitioner who will independently manage cases including unscheduled reviews respite care carer assessments and the comprehensive assessment and provision of care and support plans. The position requires the ability to adapt approaches to handle complex or unusual cases while effectively assessing and managing risks. The practitioner will collaborate with children young people families carers and communities to help them articulate their needs make informed decisions and contribute to service planning.
Responsibilities:
- Conduct unscheduled reviews and provide respite care.
- Perform carer assessments and develop comprehensive care and support plans.
- Independently manage cases adapting approaches for complex or unusual situations.
- Assess and manage risks effectively.
- Collaborate with children young people families carers and communities.
- Facilitate the expression of needs and informed decision-making.
- Contribute to service planning and development.

- Proven experience as a practitioner in a similar role.
- Ability to independently manage cases and adapt approaches.
- Strong risk assessment and management skills.
- Excellent communication and collaboration skills.
- Experience working with children young people families and carers.
- Ability to contribute to service planning and development.
