Social Worker Preparation for Adulthood

Liquid Personnel is recruiting anAdult Social Worker to join its clients Preparation for Adulthood team based in Sheffield.

What will your responsibilities be

• Transition Planning Lead holistic personcentred transition assessments for young people (1425) moving from childrens to adults services ensuring Care Act eligibility strengthsbased planning and coordinated multiagency pathways.

• Risk & Safeguarding Manage complex cases involving safeguarding concerns capacity issues or escalating needs providing professional oversight Mental Capacity Act application and robust risk management.

• MultiAgency Coordination Work closely with education health SEND commissioning and community partners to secure appropriate adultlife outcomes including housing employment independence skills and ongoing support packages.

Qualifications and Experience:

To be successful in this role you must have;

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
