3 Month Contract With A Local Authority
Role Purpose
To undertake person-centred assessments and welfare visits for Haringey residents service users and their carers in line with the Care Act 2014. The role focuses on maximizing independence through preventative interventions strengths-based approaches and collaboration with voluntary and community groups.
Main Responsibilities
Manage and prioritise workload meeting deadlines and maintaining high-quality written work.
Conduct holistic person-centred assessments and reviews focusing on strengths and preventative interventions.
Assess review and provide support to carers and families.
Ensure compliance with the Mental Capacity Act conducting capacity assessments and making Best Interest decisions where required.
Use assessments to achieve outcomes for service users and hold providers accountable.
Maintain clear accurate and up-to-date records.
Implement changes to support arrangements manage personal budgets and ensure value-for-money care packages.
Work within the framework of the Care Act 2014 to deliver accessible holistic and wellbeing-focused assessments.
Fully participate in supervision and professional development.
Apply knowledge of the Continuing Health Care framework including DST completion and joint working with health colleagues.
Collaborate with commissioning teams to maintain quality standards of providers.
Manage risks collaboratively with partners carers and providers supporting positive risk-taking.
Understand Haringey safeguarding processes statutory guidance and undertake safeguarding enquiries.
Comply with legislative and organisational frameworks while maintaining best practice.
Participate in team meetings to improve outcomes and service development.
Work proactively with professionals organisations and carers to set targets and maximise life opportunities.
Meet performance management indicators within the Assessment Service.
Understand borough resources especially voluntary and community sectors to connect service users and families to support.
Exercise responsibilities under the Health and Safety at Work Act.
Undertake mandatory and role-specific training for continuous professional development.
Requirements
Working knowledge of key legislation within health and social care.
Robust understanding of health and social care frameworks and their impact on practice.
Knowledge of social and health groups with ability to engage positively for best outcomes.
Strong written and analytical skills; outcome-focused reports with actionable solutions.
Excellent communication skills to support engagement.
Strong organisational skills and time management.
Proficiency in Microsoft Windows applications.
