TRANSITION CARE PROGRAM (TCP) COORDINATOR

An exciting opportunity exists for an experienced and motivated clinician to step into the role of Transition Care Program (TCP) Coordinator.
In this leadership position you will oversee the delivery of a high-quality Transition Care Program that supports older people at a critical point in their recovery journey bridging the gap between hospital and home or residential aged care.
You will lead a multidisciplinary team to provide short-term goal-directed and therapy-focused support for people aged 65 years and over helping them:
This is a pivotal role at the interface of acute sub-acute and community care. You will ensure safe coordinated transitions optimise functional outcomes and uphold best-practice care planning in line with the Transition Care Program guidelines.
