Where Youll Be Working

Join the dedicated Acute to Aged Related Care Services (AARCS) team at Manning Hospital where collaboration compassion and clinical excellence come together to make a real difference for older people.

AARCS plays a vital role in bridging acute and aged care supporting older adults living with dementia experiencing delirium or managing complex and chronic health needs during their hospital stay. The team provides comprehensive assessment coordinated care and personcentred support to ensure safe timely and appropriate transitions from hospital to home or residential aged care.

Working closely with medical nursing allied health and community partners AARCS fosters a supportive multidisciplinary environment that values teamwork communication and shared learning. About the Role

As a Clinical Nurse Specialist - Acute to Aged Related Care youll bring advanced clinical knowledge and compassionate leadership to the care of older people in the acute setting. Your focus will be on comprehensive assessment care coordination and transition planning helping patients move safely and smoothly between hospital home and community or residential care.

In this role you will:

Conduct holistic assessments of older patients including functional cognitive and social factors

Collaborate with the multidisciplinary team to identify risks and develop tailored transition plans

Liaise with families carers aged care facilities and community providers to support continuity of care

Facilitate referrals educate patients and carers and advocate for those living with dementia or frailty

Contribute to case conferences and quality improvement initiatives that strengthen the patient experience

Reporting to the Deputy Director of Nursing youll work alongside a skilled and caring team committed to improving outcomes and empowering older adults to maintain independence and wellbeing.

About You

Youre a compassionate and proactive nurse with strong assessment and coordination skills passionate about improving care for older people. You thrive in a multidisciplinary environment using your clinical judgment and communication skills to guide decisions and advocate for your patients.

Essential Requirements

Registered Nurse with current Authority to Practice with AHPRA
Postgraduate qualifications in Gerontology Aged Care Rehabilitation or Chronic Disease Management
Evidence of at least three years of clinical experience in the care of older people (inpatient and/or community setting)
Eligibility to drive in NSW with a willingness to travel for work purposes.
