SUMMARY DESCRIPTION OF WORK
Professionals at this level have excellent communication skills with both client and community institutions. Specific project program or office responsibilities may occur at this level. Decision-making problem solving and highly developed interpersonal skills are critical. This position functions as a fully integrated Team Member of the Assertive Community Treatment Team which is a multi-disciplinary team that assists individuals with a serious mental illness to minimize symptoms and the resulting impairment in daily functioning.
Compensation: (salary is determined by total years of relevant experience)

MINIMUM QUALIFICATIONS
- Bachelors Degree in Human Service field.
- Three years in case management vocational or skill development experience.
- Valid drivers license acceptable driving record.
Essential Duties and Responsibilities
- *Client assessment of individual skills/interests and formulation of a plan for employment/education goals.
- *Assist clients in developing skills in the areas of daily living social and interpersonal relationships and occupational capabilities.
- *Provide support and consultation to team members and community partners.
- *Maintain all contact documentation and comply with contract conditions.
- *Provide problem solving and crisis intervention as needed.
- *Coordinate services with other community agencies and resources.
- *Provide service by helping clients manage their finances.
- Displays a courteous and caring attitude at all times to the clientele volunteers and visitors of the agency.
- Cooperates and collaborates with program area staff volunteers and other Agency staff.
- *Is dependable and punctual regarding scheduling and attendance.
- Abides by all specific program and Agency procedures policies and requirements.
- Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
- Performs other program related duties as assigned.
