*Starting rate $21/hour*

The Community Engagement Navigator is a mobile personcentered communityimpact role designed for someone who believes in the power of dignity inclusion and this position youll support brain injury survivors as they rebuild skills confidence and independence. Youll empower individuals to explore meaningful community living options make informed choices and actively shape the lives they want to lead. By forming authentic trusting relationships and learning each persons story youll help connect them with and maintain connections with vital community resources including public benefits (health care housing transportation financial security food security) technology volunteerism employment and social leisure and recreation opportunities.

Youll accompany individuals and groups on community outings that build comfort and independence collaborate with an interdisciplinary team committed to personcentered care and spark social connection by helping people discover activities that bring joy and purpose. Along the way youll assess needs and wants of the individuals to develop meaningful 1:1 and group programming identify and implement support strategies that promote independence document visits progress and goals with clarity accuracy and timeliness and act as a knowledgeable guide as individuals navigate the community around them.

Minimum Education Required

High School Diploma/GED

Additional Shift Details

Monday-Friday with opportunities for weekend shifts for special events

Responsibilities

  • Responsible for the delivery of client services to include intake service planning life skills training and coordination of other services to achieve client goals.
  • Provide support and supervision to the brain injury survivor.
  • Provide transportation and training around utilizing transportation to brain injury survivors to meet the survivors service needs.
  • Document monthly progress toward identified goals.
  • Maintain ongoing communications with program/agency staff and service providers.
  • Provide crisis intervention and access emergency services to clients as needed.
  • Identify and address community integration issues for clients on caseload or as designated.

Qualifications

  • BA/BS degree preferred with 1 year experience in the field preffered. (All related volunteer internship and employment experience will be considered) (Psychology social work special education recreation therapy communication sciences and disorders sociology kinesiology/exercise science graduates and pre-med gap year candidates encouraged to apply)
  • Must be able to perform each essential duty satisfactorily.
  • Must have sensitivity to the needs of the population.
  • Strong computer knowledge.
  • Strong analytical numerical and reasoning abilities.
  • Must have excellent interpersonal skills and ability to work as part of a team.
  • Ability to read English and communicate effectively in the primary language of the program to which he/she is assigned.
  • Must hold a valid drivers license. Must have access to an operational and insured vehicle and be willing to use it to transport clients.
  • Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
