Job Description - Social Worker -II Aftercare - Full-Time Permanent
Position Summary:
Reporting to the Manager of Intake Crisis and Outpatient Services the Social Workers primary emphasis is to provide therapeutic bridging services (including individual family group and parent support) to help clients leaving intensive services maintain (or build on) the treatment progress that was achieved during their Youthdale service involvement. Clients will range in age from 10 to 24 with services either lasting up to 3 months (the traditional Aftercare service model) or for clients referred for Aftercares Dialectical Behavior Therapy (DBT) program approximately 6 months. Services will be delivered in a range of settings with an emphasis on supporting clients/families at home or in community in order to enhance accessibility and engagement.

Hours: 37.5 hours per week

Rate of pay: $39.10 per hour

Works collaboratively with the inter-professional team client families/guardians and community resources

Job Responsibilities:
- Conduct assessments including information obtained from clients family members relevant service providers and relevant documentation ensuring that assessment (and formulation) are trauma informed and incorporate knowledge around child/youth/young adult development bio/psycho/social factors motivation and risk factors.
- Building on the assessment and formulation process engage with clients and guardians/families (as per consents) and the Youthdale treatment team to plan individualized treatment goals and complete relevant documentation to capture these goals and the assessment/formulation process which informed them. Ensure that goals are reviewed/evaluated on an ongoing basis and revised as needed.
- Utilize quantitative measures as indicated to inform assessments monitor progress readiness for discharge from the service and post discharge outcomes supplemented by qualitative factors (i.e. clinical judgement).
- Provide flexible support utilizing evidence-based interventions (i.e. DBT brief solution focused therapy motivational interviewing etc.) and foundational counselling skills (i.e. skills coaching psycho-education etc.) to help clients/families maintain or build upon treatment progress put into practice what was learned while engaged in intensive services through Youthdale and to facilitate positive re-integration into community.
- Provide therapeutic skill oriented peer and family groups in collaboration with the Aftercare team and/or other Youthdale staff.
- Provide relevant support (e.g. service navigation meeting facilitation/attendance coaching/reinforcement etc.) to encourage connection to school employment social/recreational opportunities peers and/or formal mental health/addictions services as per client/family needs.
- In collaboration with client & family/guardian complete and send a package of transfer documents (e.g. Discharge Report clinical records etc.) to facilitate a smooth transfer to future service provider(s)
- Develop effective working relations and cooperate with community child and adult service providers and agencies throughout the entire
- Coordinate and provide care that is client and family centered safe timely effective efficient and
- Actively participate in administrative clinical meetings and
- Complete required documentation including Assessment and Treatment Plans Discharge Summaries outcome measures session notes and all other reports in a timely
- Attends regular individual and group
- Represents the Agency in a professional manner when interacting with families external professionals general public and community
Qualifications
- A Masters of Social Work (MSW) or equivalent education is required.
- Membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or the College of Registered Psychotherapists of Ontario (CRPO).
- Clinical experience in providing individual and family therapy to children adolescents young adults and/or their
- Clinical experience providing group services to children adolescents young adults and/or parents is considered an
- Experience with using a variety of evidence-based/ trauma informed treatment approaches including DBT Brief-Solution Focused Therapy Family Therapy and other appropriate modalities.
- Experience working with inter-professional and residential teams is considered an
- In alignment with regulatory college expectations maintains active self-reflection in regard to clinical strengths and areas for learning and flags any professional development needs to the appropriate Manager.
- Sensitivity to and ability to work with a diverse client and staff
