The Role
As our Care Partner Support at Home youll be the architect of personalised care journeys for our valued clients. This is a strategic coordination role requiring strong industry knowledge sound judgement and the ability to adapt as the aged care landscape continues to evolve. Youll:
- Be the primary advocate and point of contact for clients across Support at Home Fee for Service and Brokerage programs
- Design and manage comprehensive individualised care plans aligned with the Aged Care Act 2024 and Strengthened Aged Care Quality Standards
- Collaborate closely with our Clinical and Allied Health teams to deliver exceptional wellness and reablement outcomes
- Strategically manage quarterly budgets maximising utilisation through proactive planning and innovative service delivery
- Champion client choice independence dignity and rights in every interaction
- Navigate ongoing sector reform with confidence contributing to continuous improvement across the team
This role requires someone who brings both experience and adaptability. Youll need:
- Demonstrated coordination experience in aged care preferably in-home or community services this is essential not negotiable
- Resilience and comfort with change youve worked through sector reforms shifting funding models or organisational change and youve thrived not just survived
- Certificate IV in Aged Care Community Services or Individual Support and/or a relevant tertiary qualification(a clinical background is highly regarded)
- Strong understanding of aged care funding frameworks particularly Home Care Packages and the Support at Home program
- Excellent communication and relationship-building skills with clients families and multidisciplinary teams
- Sound judgement initiative and the ability to manage competing priorities
- A genuine commitment to enhancing the lives of older Australians
- Current National Police Clearance (within three years)
- Australian citizenship or valid visa with full working rights
