The role is part of Islington Provider Services Day Opportunities and Hubs focusing on empowering individuals to live independently by leveraging their strengths. The services cater to adults over 18 with diverse needs including learning disabilities physical and sensory needs mental health issues and older adults. The service is undergoing transformation to become more multi-agency with potential changes aligned with service development. The position involves providing out-of-hours social activities and fostering friendships for individuals carers and families. Flexibility across Provider Services is required to meet service needs. The role supports individuals in accordance with personalized plans emphasizing independence and collaboration with health and social care professionals.
Responsibilities:
- Deliver care and support aligned with personalized support plans.
- Perform personal care domestic tasks and facilitate independent living skills.
- Work in an enabling manner to maximize independence.
- Collaborate closely with health and social care professionals involved in care planning.
- Report directly to the Team Leader or Assistant Manager.
- Exemplify best practices in strengths-based and person-centered approaches.
- Connect individuals with informal community-based support.
- Engage in partnership with residents carers community organizations and professionals.
- Participate in occasional weekend or evening events on a voluntary basis.
