Weekend Requirement: One (1) weekend per four-week schedule
Hourly Rate: $31.94 $43.92 (based on experience)
Evening Differential: $2.50/hour
Weekend Differential: $2.00/hour
(Differentials are stackable up to $4.50/hour)
Sign-On Bonus: $10000
Paid in five (5) installments over two (2) years
A large acute-care hospital in Panama City Florida is seeking an experienced Registered Nurse (RN) Case Manager to coordinate patient care assess clinical and psychosocial needs and promote the efficient use of hospital resources.
This role supports interdisciplinary collaboration improves patient throughput and facilitates safe effective transitions across levels of care while ensuring compliance with regulatory accreditation and quality standards.
Assess patients medical psychosocial and discharge planning needs
Develop and coordinate individualized case management and care transition plans
Lead and participate in interdisciplinary care team discussions
Evaluate admissions observation status and medical necessity
Perform utilization reviews and communicate with third-party payers
Coordinate safe timely and effective discharge planning
Identify barriers to care and recommend or implement process improvements
Educate patients and families and connect them with appropriate community resources
Maintain accurate and timely documentation of care coordination activities
Ensure compliance with regulatory accreditation and hospital quality standards
Associate Degree or Diploma in Nursing (required)
Bachelor of Science in Nursing (BSN) preferred
Active Florida RN license required
Compact RN license accepted with Florida licensure obtained within 90 days
Minimum 3 years of acute care case management experience OR
Minimum 3 years of RN experience in one or more of the following areas:
Medical-Surgical
Telemetry
Neurology
Intensive Care Unit (ICU)
Progressive Care Unit (PCU)
Emergency Department
Case Management certification
Nursing specialty certification
Utilization Review certification
