JOB DEFINITION:
The primary responsibility of the Registered Nurse is to maintain the security care use and disposal of all medication held by the program maintain medical equipment in the clinic administer medications to patients conduct basic physical health screenings and coordinate care between physical behavioral healthcare providers. The Registered Nurse provides direct patient care and implements treatment plans under the direction of the Nurse Manager and medical staff following established clinical guidelines policies and procedures.
EDUCATIONAL REQUIREMENTS:
Must be currently licensed as a Registered Nurse by the NY State Department of Education and possess an Associates (if awarded prior to December 18 2017) Bachelors or Masters Degree in Nursing from an accredited program*.
EXPERIENCE REQUIRED:
Minimum of 1-year clinical experience with psychotropic medications mental health & substance use disorders and comorbid medical conditions. Additional community-based nursing experience preferred.
DUTIES AND RESPONSIBILITIES:
- Responsible for the coordination of client services to include scheduling of outside clinic appointments lab appointments obtaining random urine analysis (UA) intramuscular (IM) and subcutaneous injection administration when needed
- Conducts medication reconciliation when appropriate or during transitions of care.
- Conducts vitals and health assessments.
- Conduct basic physical exams on patients comprising of taking vital signs measuring weight and height pulse pulse oximetry respirations BMI and providing appropriate education and referrals as indicated.
- Review OMH Health Screenings with clients entering treatment to assess engagement in activities related to managing and improving physical and behavioral health conditions.
- Accept maintain a log secure and discard medications in accordance with accepted protocols.
- Maintain inventory of all medications received in the ICRS program.
- Acts as liaison in conjunction with the prescribers to pharmaceutical companies Patient Assistance Programs for the purpose of securing prior authorizations medications and for clients in financial need.
- Reviews medication management and injection appointment schedules to ensure that program maintains sufficient quantities of injectable psychotropic medications to meet client needs.
- Establish communication channels and facilitate coordination of care activities among clients medical behavioral and specialty healthcare providers.
- Provide education to clients related to the legal and ethical issues involved in data sharing in order to maximize continuity of care promote health literacy and confidence and ensure that information is shared in accordance with an individuals wishes.
- Check monthly the working order of defibrillator oxygen equipment and replenish first aid kit supplies as needed in the front office.
- Participate in clinical treatment team meetings and support colleagues in order to promote a team-based approach to care.
- Participate in facilitating group sessions on health topics with social workers.
- Order medical supplies as needed for the clinic.
- Complete trainings as required or requested by the agency or its governing entities.
- Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
- Complete QI Training during onboarding and participate in QI refresher training as needed.
