Position Overview

We are seeking a licensed Physical Therapist for a contract assignment in a Skilled Nursing Facility setting. The therapist will evaluate and treat patients to improve mobility restore function reduce pain and prevent disability following injury illness or surgery.

Responsibilities

Conduct patient evaluations and develop individualized treatment plans
Implement therapeutic exercises and rehabilitation programs
Monitor patient progress and adjust treatment plans as needed
Educate patients and caregivers on therapy techniques and discharge planning
Collaborate with interdisciplinary healthcare team members
Maintain accurate and timely clinical documentation

Minimum Qualifications

Active state Physical Therapist license (must be current)
Experience in a Skilled Nursing Facility preferred
New graduates considered (less than 1 year experience accepted)
Strong communication and patient care skills
